Position: Outreach Case Manager, HIV Team
Hours: Full Time

Reports to: Director, HIV Services
Essential Duties:
The outreach case manager will regularly visit shelters, congregate living facilities, the
street and apartments to connect with patients who may have fallen out of care or are at
risk of falling out of care. The outreach CM will help to reduce barriers for people to
access services, both medical and non medical to help improve their lives. This may
include transportation vouchers, reminders via phone calls, visits or accompanying the
patient to the appointment. As part of the HIV medical team, the outreach case
manager will meet with new patients to assess his or her financial and psycho-social
needs. She/he will develop and help the patient carry out a plan to achieve success in
housing, substance abuse treatment and access to other services. The case manager
will meet with other members of the team regularly to exchange information about the
needs of particular patients. She/he will also be available on an ongoing basis to
provide support and referrals as needed to the established patients. The case manager
will be a liaison between the many AIDS service organizations and the HIV medical
team of BHCHP. She/he will also be available on an ongoing basis to patients for crisis
support and psycho-social support.
Qualifications:
· HIV case management, Education and/or Counseling Experience Required
· Bilingual in Spanish and English preferred
· Bachelors Degree preferred
· Demonstrated interest in working with an undeserved population
Physical and Mental Requirements
· Must be able to work cooperatively as a member of a health care team which uses a collaborative
approach and have the ability to deal positively with a busy and stressful workload and work schedule
involving multiple sites.
· Must be self motivated and have the ability to work independently.
· Must have the physical ability to travel and to access assigned sites as well as the physical ability to
utilize computers and telephones.
