Job Title: Senior Family Partner (Parent to offer Peer Support) Category: Direct Care - Wraparound Family Services Career Center: BAMSI Career Site Type of Assignment: Permanent Position Type: Full Time Location: Wraparound Family Services City/Town: Brockton and Surrounding Towns State/Province: Massachusetts Pay Rate From: 17 Pay Rate To: 19 Pay Rate Unit: Hour Job Information: Qualifications:. Experience as a caregiver of a youth with special needs and preferably a youth with mental health needs and have experience securing appropriate services and resources.. Bachelor's degree in a human services field from an accredited university and one (1) year of experience working with the target population; or. Associate's degree in a human service field from an accredited school and one (1) year of experience working with children/adolescents/transition age youth; or high school diploma or GED and a minimum of two (2) years of experience working with children/adolescents/transition age youth; and. Must have experience working collaboratively with state agencies, consumer advocacy groups, and/or behavioral health outpatient facilities. Prior facilitation and budgeting experience necessary.. Computer experience necessary, especially with Microsoft Office suite.. Must possess excellent communication (verbal and written), organizational and interpersonal skills.. Must have knowledge of community resources in designated work area and the ability to accept Families, children and youth unconditionally without judgment or blame for the problems they face.. Ability to constructively negotiate the system and communicate effectively with service providers and administrators.. Must be flexible and able to respond to Families under stress and in crisis.. Senior Family Partners must possess a current and valid driver's license and automobile. The Senior Family Partner ensures the Family's voice is heard throughout the wraparound process and acts as a role model for Parents and Caregivers in interacting with community providers and agencies.Responsibilities:1. Provides orientation, ongoing training, job shadowing, weekly individual and group supervision, performance evaluations and support to Family Partners and promotes positive interactions with Families, providers and organizations. Completes Team Observation Measure (TOMs) and other fidelity measures2. Assigns Families to Family Partners and assists Families in understanding the scope and services of the Community Service Agency program and the family's role, rights, and responsibilities in the program3. Assists Senior Care Managers with intake process and assists Family Partners in organizing and facilitating caregiver peer support groups, and in developing peer leadership among caregivers4. Participates in the Systems of Care Committee and collaborates with the Community Service Agency Program Director, other Senior Family Partners and Senior Care Managers5. Advocates for and secures necessary community resources that benefit Families in the service area and fosters child and family participation in the development and understanding of the Individual Care Plan goals and interventions6. Provides coaching, training and support to Parents/Caregivers as relates to interaction with their child and in understanding how to advocate for services and resources to meet their child's needs7. Participates in the child's Individual Care Planning Team (ICPT) and facilitates team meetings as needed. Provides timely, accurate documentation as required by agency and program8. Monitors Families' satisfaction with the Community Service Agency services, monitors the established goals for families and assists Parents/Caregivers in meeting care planning and/or treatment goals9. Tracks program referrals, intakes and census. Reviews family files, weekly progress notes and strengths inventories for quality assurance and ensures program compliance.10. Obtains familiarity with the CANS assessment tool and is able to relay its use to families11. Advocates with the Family and Child within the education system by attending school meetings and participating in the development of any school plans/IEPs; and within other systems such as the courts, state agencies and community organizations12. Is available to Parents/Caregivers by phone or at the family home during non-business hours and provides transportation to Parents/Caregivers as needed13. Assists Parents/Caregivers in recognizing the importance of self-care as a vital component of their parenting, and helping them connect with support in the area of self-care14. Conducts regular face-to-face meetings with Parents/Caregivers as required by the program, actively participates in transition planning with Families and assists in the development of community-based natural supports15. Performs duties to reflect agency/funding source policies and procedures and reasonable safety standards. Universal/standard precautions must be utilized and training obtained in areas that constitute risk.16.
